Guest Checkout does not work

Hi. We have our "Guest Checkout" set to "On".
Simply put, it does not work.
It still gives us the "Register" tab and the "New Customer" section, asking new customers to enter email, password, confirm password when they checkout.

What am i missing? What the heck is happening? Thank you.

    We have the "Guest Checkout" extension, and it's reported "Installed and Active", but the icon is broken.
    Could this have something to do with our problem? How to fix..?
    Tried Deactivating and Activating again.

    I don't know how this was installed... if the previous developer just copied it over, maybe that would make it not work..? How do these extensions normally get installed?

    Also, how do i know what type of license we have? Maybe we have an unlimited license, maybe a single user one, but i can't tell.

    Regarding the Guest Checkout problem...
    Can you please ensure that you have the latest version of the Guest Checkout extension plugin for the Shopping Cart plugin?

    You can get the latest here: http://tribulant.com/downloads/ . And that is also where you can see if you have a single or unlimited domains license.
