[Solved] Manage Subscriptions Form missing

I removed the Manage Subscriptions form by mistake and now cannot locate it again.  It does not appear in the forms tab.  Please let me know where it resides.

You can create a new page and add the manage subscription shortcode to it [newsletters_management] 

You can then go to Newsletters>Configuration-Subscribers- Subscriber Management Section - Management Post ID and make sure the correct page/post id is filled in. http://www.screencast.com/t/scmDLuSojVd
